No, but the hospital will provide a physician for you. If you prefer them to be physically mailed, please provide a preaddressed stamped envelope. Please send the forms via email or by fax or simply drop off at the office and let us know how you would like the form returned to you. Your child must have completed an annual well checkup with the PCP within the last year. Any form needing an immediate same day turnaround can be completed at the office for an express processing fee of $30. All forms will be completed free of charge.
